In preparation for your appointment, please register as  a user on this website and complete the Medical History form found in the Health Forms tab.  This will only take a few minutes and will save you time at your office visit.  You may also want to complete a Symptom Assessment, which will help your doctor get a better sense of your problem.

Once you are a registered user, you can access our interactive features like the symptom assessments, appointment requests, refill your prescriptions, update your information, or pay your bill on line.  Save time and money with our web site.

When you come for your visit, you will want to be sure to bring your insurance card, a picture ID and your copayment.  If you do not have insurance, a $150 deposit is required at check in and will be applied to the cost of your visit at check out.  If you have any questions, don't hesitate to contact us.
